GLIDE is hiring a Case Manager I for their Health Systems Navigation team to provide comprehensive, client-centered case management. This role focuses on facilitating access to health services for individuals who use substances.
You are a compassionate individual with a strong commitment to social justice and a desire to empower individuals facing systemic injustices. You have a background in case management or a related field, and you understand the importance of harm reduction in supporting individuals who use substances. You are skilled in conducting outreach and building rapport with clients, ensuring they feel supported and understood throughout their journey. You are dedicated to helping clients achieve their goals, whether that involves improving their health, securing housing, or accessing social services. You possess excellent communication skills, allowing you to effectively collaborate with various stakeholders, including healthcare providers and community organizations.
Experience working with marginalized populations or in a healthcare setting is a plus. Familiarity with health services related to substance use treatment, behavioral health, and HIV/HCV/STI prevention will enhance your ability to support clients effectively. You are adaptable and open to learning, ready to embrace the challenges that come with this rewarding role.
As a Case Manager I at GLIDE, you will provide comprehensive, client-centered case management with a focus on harm reduction. You will manage a dedicated caseload, developing individualized care plans that prioritize client-identified goals.
