Biofourmis is hiring a Telehealth Nurse Practitioner to manage a panel of patients and contribute to a telemedicine program focused on cardiology and heart failure. This role requires experience in overseeing complex chronic care programs and a commitment to health education.
You are a dedicated Nurse Practitioner with a specialization in cardiology and heart failure, bringing experience in managing complex chronic care programs. You are bilingual, fluent in both English and Spanish, which enhances your ability to connect with a diverse patient population. Your background includes a strong commitment to health education and promotion techniques, ensuring that patients receive the best possible guidance in their health journeys. You are comfortable working in a HIPAA compliant environment and are motivated to leverage technology to improve patient outcomes. You have experience using multiple electronic medical records (EMRs) and various communication applications, including Google products, which allows you to efficiently manage patient care remotely.
In this role, you will virtually evaluate patients entering the telemedicine program, assisting in the creation of personalized care plans based on specific patient needs and established protocols. You will respond to alerts escalated from the nursing team regarding deviations in patients' vitals and physiological data, ensuring timely interventions. Your responsibilities will include managing a growing panel of patients, providing ongoing support and education to help them navigate their health challenges. You will collaborate with a multidisciplinary team to enhance the telemedicine program, utilizing first-in-class medication management titration algorithms to optimize patient care. Your work will be crucial in driving the growth and development of the program, as you implement best practices in chronic care management and patient engagement.
