Nightingale is seeking a Primary Care Advanced Practitioner to provide comprehensive primary care services in a supportive environment. You'll manage chronic conditions and promote wellness through education and preventive measures. This role requires collaboration within a close-knit care team.
You are an experienced Primary Care Advanced Practitioner, either a Nurse Practitioner (NP) or Physician Assistant (PA), with a strong commitment to patient-centered care. You understand the importance of building long-term relationships with patients and are dedicated to providing comprehensive primary care services. You thrive in a collaborative environment and value the autonomy that comes with being an integral member of a healthcare team. Your focus is on preventive medicine, and you are skilled in managing chronic conditions while educating patients about their health.
In this role, you will provide direct patient care in an outpatient setting, conducting routine office visits and addressing acute medical concerns. You will perform annual wellness visits and health risk assessments, ensuring that patients receive the preventive care they need.
