GLIDE is seeking a Health Access Navigator to support individuals with substance use and co-occurring disorders. You'll provide health education, referrals, and harm reduction supplies in San Francisco. This role is ideal for those passionate about social justice and community health.
You are dedicated to social justice and have a strong desire to support individuals facing systemic injustices. You understand the challenges faced by marginalized communities, particularly those dealing with substance use and homelessness. Your empathetic nature allows you to connect with clients and provide the necessary support they need to navigate health services. You are comfortable working in a collaborative environment and are willing to engage with various community and city providers to advocate for underserved populations. You possess strong communication skills, enabling you to educate clients about health services and harm reduction strategies effectively.
As a Health Access Navigator at GLIDE, you will play a crucial role in connecting individuals with essential health services and resources. You will provide harm reduction supplies and proactive counseling to clients with a history of substance use. Your responsibilities will include offering health education and information, facilitating referrals, and ensuring clients have access to the support they need to overcome barriers to care. You will work closely with the Syringe Access Services team to deliver comprehensive health services and engage clients in recovery and health improvement activities. Your role will involve advocating for clients and collaborating with community partners to enhance the overall wellness of the community.
