GLIDE is seeking a Case Manager I for their Medications for Opioid Use Disorder program. You'll work with clients to connect them to essential resources and support their recovery journey. This role requires a commitment to social justice and community empowerment.
You are dedicated to social justice and have a strong desire to support individuals facing opioid use disorder. You possess excellent communication skills and are able to build rapport with clients from diverse backgrounds. You understand the importance of providing comprehensive support and resources to help clients achieve stability and self-sufficiency. You are empathetic and patient, recognizing the challenges faced by those in recovery. You are organized and capable of managing a caseload effectively, ensuring that each client receives the attention and resources they need. You are committed to continuous learning and staying informed about best practices in addiction treatment and recovery.
As a Case Manager I, you will work closely with clients who have experience with opioid use disorder, helping them access medications such as methadone, suboxone, and naltrexone. You will develop and maintain a caseload of 20 to 30 participants, ensuring they have the knowledge and resources necessary for treatment adherence and stabilization. You will conduct assessments to identify clients' needs and connect them to Glide Foundation resources, including housing assistance and mental health services. You will provide counseling and intervention to support clients in their recovery journey, emphasizing the importance of medication adherence and overall well-being. You will collaborate with other team members to create a supportive environment that fosters positive health outcomes for clients. You will document client progress and maintain accurate records to track treatment adherence and outcomes.
