Oscar is hiring a Case Management Nurse to join their Case Management team. You'll educate members on health outcomes and assist with care transitions. This role is remote and open to candidates in Arizona, Florida, Georgia, or Texas.
You are a dedicated Case Management Nurse with a passion for improving health outcomes for members. You have experience in coordinating care across various settings, including inpatient, outpatient, and home care. You possess strong communication skills, allowing you to effectively educate members about their health conditions and assist them in navigating their care plans. You are comfortable working remotely and are open to occasional travel for team meetings and company events.
Experience in a health insurance environment or familiarity with case management processes is a plus. You are proactive in reaching out to members undergoing health challenges and have a knack for developing personalized care plans that cater to their unique needs.
In this role, you will be responsible for educating members on improving their health outcomes and assisting with transitions from various care settings. You will actively reach out to members facing difficult health challenges, developing tailored care plans to support their recovery. You will also communicate with hospital case managers to assist with discharge planning, ensuring a smooth transition for members returning home. Your role will involve collaborating with support teams to ensure exceptional care for our members, participating in process improvement initiatives, and contributing to pilot programs as they arise. You will report to the Associate Director, Clinical, and work closely with other team members to enhance the overall member experience.
Oscar offers a competitive hourly wage ranging from $39.28 to $45.94, along with employee benefits and a monthly vacation accrual rate of 15 days per year.
