Who you are

You are an experienced healthcare professional with a strong background in delivering patient care, particularly in a virtual setting. You have a proven track record of managing teams and coordinating care among multidisciplinary groups. Your ability to communicate effectively with patients and team members is essential, as you will be guiding a group of Advanced Practice Providers (APPs) in delivering high-quality care. You are adaptable and comfortable working in a remote environment, ensuring that patient needs are met efficiently and compassionately.

Desirable

Experience in telemedicine or virtual care models is a plus, as well as familiarity with innovative healthcare delivery systems. You may also have a background in managing patient panels and coordinating care across various healthcare providers.

What you'll do

In this role, you will be responsible for providing direct patient care primarily through telemedicine, utilizing video, phone, and messaging to connect with patients. You will manage a panel of patients, ensuring they receive comprehensive care tailored to their needs. As a Lead APP, you will supervise a team of Advanced Practice Providers, providing guidance and support as they deliver care to their patient panels. You will collaborate closely with Registered Nurses and Medical Assistants to ensure a seamless care experience for patients. Your role will involve coordinating in-person care when necessary, ensuring that patients receive the best possible outcomes.

You will report to the Medical Director of Virtual Primary Care, contributing to the development and implementation of innovative care models that prioritize patient satisfaction and health outcomes. Your leadership will be crucial in fostering a collaborative environment among team members, promoting a culture of excellence in patient care.
